Lower limb leg discrepancy is classified as congenital or acquired, consequent to trauma or bone diseases, and is usually treated with external fixation (Ilizarov technique). Recently, extending intramedullary magnetic nails allow the lengthening with a minimally invasive technique.
Nowadays, bone reconstruction after lower limbs bone tumors in growing patients is challenging. Aims of this surgery are to maintain the length of the limbs and a good joint range of motion. Even if the bone reconstruction with biological devices (homoplastic or autoplastic bone grafts) maintains bone stock in young patients, it doesn’t prevent limb discrepancy.
The purpose of the update is to compare the different lengthening techniques, discussing indications, problems and results.
